ACL rehab.

ACL rehabilitation is one of the most complex and consequential pathways in sports physiotherapy. Return-to-sport rates after ACL reconstruction are disappointingly low industry-wide, largely because rehabilitation is cut short of true functional readiness. At Motion Theory, ACL rehab is criterion-based: you progress when objective tests confirm readiness, not when a calendar says it is time.

Who It's For

Athletes and active individuals following ACL reconstruction including BPTB, hamstring, or allograft graft types, as well as those managing ACL deficiency conservatively. Applies to all sports and activity levels.

What We Assess

Your assessment maps your current position on the ACL rehab timeline including strength deficits, range of motion, swelling, and functional testing. We establish your Limb Symmetry Index and set clear criteria for each phase transition.

Treatment Approach

ACL rehabilitation at Motion Theory follows a criterion-based protocol with five phases: acute management, strength restoration, running and change of direction, return to training, and return to competition. No phase is skipped. Quadriceps strength at 90 percent LSI is the primary clearance criterion, alongside hop testing and psychological readiness.

Recovery Pathway

Months 0-3

Swelling, Range & Quad Activation

Restore full knee extension, manage swelling, and rebuild quadriceps activation, the foundation everything else depends on.

Months 3-6

Strength, Running & Agility

Bilateral and unilateral strength development, return to straight-line running when quad LSI exceeds 60 percent, and progressive cutting mechanics.

Months 6-9+

Return to Sport Criteria

Quad and hamstring LSI above 90 percent, single-leg hop testing, psychological readiness, and sport-specific demand simulation before clearance.

Common Questions

When can I return to sport after ACL reconstruction?

Return to contact sport before 9 months with less than 90 percent quad symmetry carries a 4 to 6 times increased re-injury risk. We use objective strength and hop testing, not calendar time, to guide return decisions.

Do I need surgery for an ACL tear?

Not always. Young active athletes typically require reconstruction for return to cutting sports. However, many individuals, particularly older or less active patients, can return to their goals with structured conservative rehabilitation.

My knee feels fine at 4 months. Can I return to sport?

Pain absence does not equal readiness. Quadriceps strength typically remains significantly deficient at 4 months regardless of symptoms. We use objective testing, not perceived readiness, to guide this decision.
