Motion Theory Rehabilitation Inc. is a private, multi-disciplinary rehabilitation clinic located at 1367 West Broadway, Vancouver, BC V6H 1A9. We provide physiotherapy, kinesiology, massage therapy, chiropractic, and acupuncture services. We are a registered private body under PIPA and independently owned and operated.

2. Information We Collect

Clinical information: Name, date of birth, health history, assessment findings, treatment records, medications, and insurance information (ICBC claim numbers, extended health plan details, WorkSafeBC file numbers).

Contact information: Phone number, email address, mailing address, and emergency contact details.

Website data: When you use our website, we may collect non-personally identifiable data such as browser type, referring URL, and pages visited, for the purpose of improving our website. We do not use tracking cookies for advertising purposes.

Form submissions: Information you submit via our contact, booking, careers, or referral forms is stored securely and used solely for the purpose for which it was submitted.

3. How We Use Your Information

We use your personal information to:

  • Provide, coordinate, and document clinical care
  • Process insurance claims and direct billing (ICBC, WorkSafeBC, extended health)

Clinical records are retained for a minimum of 10 years from the date of last contact, or until a minor patient turns 19 years of age, whichever is longer, in accordance with BC College of Physiotherapists of BC and other regulatory body standards. Records are stored securely in electronic format using encrypted, access-controlled systems.

7. Security

We implement industry-standard administrative, technical, and physical safeguards to protect your personal information from unauthorized access, use, disclosure, alteration, or destruction. Our electronic health records system is encrypted and access is restricted to authorized clinic staff on a need-to-know basis.

8. Access and Correction

You have the right to request access to your personal information held by Motion Theory and to request corrections where you believe information is inaccurate or incomplete. Requests must be made in writing. We will respond within 30 business days. A nominal fee may apply for record retrieval in certain circumstances.
